Applying to Our Programs
Applicants to our programs have the option of applying online or downloading an application to print and mail.
A completed application includes:
- Application for Training
- Medical History and Physical Examination Form
- Eye Care Professional Form
- Consent to Release Information Form
- Video demonstrating independent travel skills

- Complete our online application
- Download the Online Application Packet (ZIP file), which includes the required medical, eye care and consent forms.
- Send the completed Application Packet, along with a recent photo and your video, to: Leader Dogs for the Blind, Consumer Services, P.O. Box 5000, Rochester, Michigan 48308-5000.
