Applying to Our Programs

Applicants to our programs have the option of applying online or downloading an application to print and mail.

A completed application includes:

• Application for Training
• Medical History and Physical Examination Form
• Eye Care Professional Form
• Consent to Release Information Form
• Video demonstrating independent travel skills

Important announcement about the online application

NOTE: To apply for our 2013 Summer Experience, please visit the Summer Experience page for program information or download the Summer Experience application.

1. Complete our online application.
2. Download the Online Application Packet (ZIP file), which includes the required medical, eye care and consent forms.
3. Send the completed Application Packet, along with a recent photo and your video, to: Leader Dogs for the Blind, Consumer Services, P.O. Box 5000, Rochester, Michigan 48308-5000.

Apply by Mail

1. Download and complete an application packet:
   • Guide Dog and Accelerated Mobility Program Application Packet (ZIP file)
2. Send the completed Application Packet, along with a recent photo and your video, to: Leader Dogs for the Blind, Consumer Services, P.O. Box 5000, Rochester, Michigan 48308-5000.
