Proof of Canine Vaccination

Service dogs are always welcome to join you on a visit to the Leader Dogs for the Blind’s campus. To protect the health of our on-campus canines, all visiting service dogs (including working Leader Dogs) traveling to the Leader Dog campus must have prior authorization.

Please fill out the form below and upload a PDF or clear image of your dog’s vaccination record at least two weeks prior to your campus visit. Please note that Leader Dog does not keep vaccination records of visiting service dogs on file, so you will need to resubmit your dog’s records before a return visit.

Required vaccinations include: Rabies, Distemper/Parvo, Leptospirosis, Bordetella (Kennel Cough), *we do not require Canine Influenza Vaccine but highly recommend.
