Proof of Canine Vaccination

Service dogs are always welcome to join you on a visit to the Leader Dogs for the Blind’s campus. To protect the health of our on-campus canines, all visiting service dogs (including working Leader Dogs) traveling to the Leader Dog campus must have prior authorization.

Please fill out the form below and upload a PDF or clear image of your dog’s vaccination record at least two weeks prior to your campus visit. Please note that Leader Dog does not keep vaccination records of visiting service dogs on file, so you will need to resubmit your dog’s records before a return visit.

I hereby certify that my service dog will be current on all the following vaccines during my visit to the Leader Dog campus.* Bivalent Canine Influenza*, Kennel Cough Vaccine, Leptospirosis, Rabies and Distemper/Parvo. *The Bivalent Canine Influenza vaccination series takes four to six weeks to complete. At least two weeks must elapse after the first dose before the second dose can be administered. The dog will not be considered vaccinated for an additional two weeks after the last dose is given.(Required)
In addition to the above vaccines, I hereby certify that my service dog has had a negative fecal test for ova and endoparasites within the last year.(Required)
