Proof of Vaccination for Visiting Service Dogs

To protect the health of our on-campus canines, all dogs travelling to Leader Dogs for the Blind’s campus must have prior authorization.

Please fill out the form below and upload a PDF or clear image of your dog's vaccination record. You can also download the form and fax with current veterinary shot record to: Leader Dogs for the Blind at 866-504-1121 no later than 2 weeks before campus visit. Please note that Leader Dog does not keep vaccination records of visiting service dogs on file, so you will need to resubmit your dog's records before a return visit.

After approval, guests will be mailed an identifying tag that must be displayed on the dog’s collar. Any dog outside of compliance will not be allowed on campus.

Leader Dog does not offer boarding facilities on or off site.

For current Leader Dog clients scheduled to visit campus, please contact the Leader Dog veterinary department at 888-777-5332 for financial assistance if necessary.

Proof of Vaccination

  • *The Bivalent Canine Influenza vaccination series takes 4–6 weeks to complete. At least 2 weeks must elapse after the first dose before the second dose can be administered. The dog will not be considered vaccinated for an additional two weeks after the last dose is given.
