Application Next Steps

Thank you for submitting part one of your application! A member of the Leader Dog client services team will be in touch with you shortly to inform you of the additional information needed for your application. Or you may choose to get started right away by downloading and completing the required forms below (all forms will open in PDF):

Please note that the application process can take several months to complete. Leader Dogs for the Blind relies on outside references based on the information provided in your application, such as past O&M instructors, mental health providers, past guide dog organizations, personal references and background check requests. The time frame for applications to be completed can depend on the speed of paperwork submitted by these providers and references. The Waiver, Release of Liability, Indemnity and Consent to Use and Release Information form above allows Leader Dog to contact these outside references, so you can help decrease length of the application process by returning all requested documentation above as quickly as possible.

Once all necessary information and forms have been obtained, the application will be brought to the Leader Dogs for the Blind Admissions Committee for review. Once a decision has been made, we will contact you.

If you are not accepted for training, a letter will be sent explaining the reason(s) for the denial. In most of these cases, an applicant is denied because he or she has not met a basic prerequisite.
